By default, a medical history form attached to a Reminder Campaign is sent for every appointment type.
To send the medical history form for examinations only, you must create a mailing group for examination appointments, exclude those appointments from the general reminder campaign, and then add a dedicated Reminder Channel for them.
Note: These steps can only be completed with a Practice Manager account.
To apply medical history forms to examination appointments only:
Part 1 - Create a Mailing Group for Examination Appointments:
- Log in to Patient Bridge using a Practice Manager account.
- From the menu bar, select Communications.
- Click Reminders.
- Click Manage Groups.
- Click Add New Group.
- Enter a Group Name — for example, Examination Group.
- Click Any under Available Treatments and select the appointment types that require a medical history form.
- Click Any under Available Patient Statuses and select Current.
- Leave Outbound Format Only set to Please Select....
- Click Add Filter.
- Click Save Group.
Part 2 - Exclude those Appointments from the General Reminder Campaign:
- From the menu bar, select Admin.
- Click Manage Treatment Filters.
- Click Add Filter.
- For Filter Type, select Reminders.
- For Sub Type, select General.
- Under Available Treatments, select the same examination treatment types you added to the mailing group.
Note: This exclusion is essential. Without it, patients will receive both the general reminder and the examination reminder. - Click Save.
Part 3 - Add a Reminder Channel for Examinations:
- From the menu bar, select Communications.
- Click Reminders.
- Click Configure Reminders.
- From the Actions drop-down list for the current reminder channel, select Manage Reminder Channels.
- Click Add Reminder Channel.
- For Campaign Name, enter Examinations Only.
- Set VWR Collection to the medical history form.
- Set Outbound Format to SMS.
- Select the message template to use, or click +Add New Message Template to create one.
- Under Mailing Group, select the examination group you created earlier.
- Click Save.
Part 4 - Set the Schedule and Enable the Channel:
- From the Actions drop-down list for the new reminder channel, select Configure Schedule.
- Click Add Reminder Schedule.
- For Name, enter 48 Hours.
- Set Period to 48 hours.
- Set Pre Or Post to Pre-Appointment.
- Click Save, then click Back to Reminder Channels.
- From the Actions drop-down list for the new channel, select Enable Automation.
- Click Back to Reminder Channels.
- Check the original reminder channel: From the Actions drop-down list for the original reminder channel, select Edit.
- Confirm that no VWR Collection is applied.
Note: Confirm that the message template does not contain a VWR link. If it does, patients following the link will reach a blank page. Create a new reminder template without a VWR link if necessary. - Click Save.
- Confirm that all reminders are enabled.
