You can create a PSR (US) or BPE (UK) for a patient from either the Perio Chart (Patient > Clinical > Perio Chart) or the Hard Tissue Chart (US)/Dental Chart (UK) (Patient > Clinical > Hard Tissue Chart or Dental Chart).

- From the Patient tab, select Clinical > Perio Chart or Clinical > Hard Tissue Chart/Dental Chart.
- On the top of the screen, next to PSR or BPE, click New Exam.

You can input the applicable score, per sextant, including the ability to flag the sextant for any additional abnormal findings or furcation. - Fill in the Periodontal Screening and Recording or Basic Periodontal Exam information.
Note: By default the Exam Date is set to the current day; to change it, click the Calendar icon. - Click Save Exam.
Note: You can click Edit Exam to update the current exam or click the down arrow in the bottom-right corner of the top-right tile to view historical exams. The exam is also recorded in the patient's clinical notes.
See How to Create a Perio Chart for more information.
Basic Periodontal Exam (BPE) and NHS Claims (UK Only)
A patient's highest BPE sextant score is automatically prepopulated in the NHS claim at the time of claim creation (Course of Treatment acceptance). When the patient's treatment plan is completed (unless previously entered or overwritten by the user), the highest BPE sextant score is updated if a new exam has been completed or an existing has been updated.
Note: This value is updated if the Course of Treatment is uncompleted and recompleted. See How to Reopen Completed NHS Treatment Plans for more information.
The highest BPE sextant score is displayed under the Clinical Data Set / DMF section of an NHS claim. You can manually override the highest BPE score during claim submission. See How to Update an NHS Claim for more information.
