ACORN Assessments

The ACORN assessment (Assessment of Clinical Oral Risks & Needs) is a structured clinical tool used to evaluate a patient’s oral health risks and needs. It assists dental teams in planning appropriate preventive and treatment care based on individual clinical findings. This assessment is specific to NHS Wales.

Viewing an ACORN Assessment

1. Open the NHS patient record.

2. Go to Clinical > Dental Chart.

3. Under Mode, select Planning.

4. Click the tree icon .

5. Use the Previous drop-down to access earlier assessments.

Creating an ACORN Assessment

1. Open the NHS patient record.

2. Go to Clinical > Dental Chart.

3. Under Mode, select Planning.

4. Click the tree icon .

5. Complete the assessment fields as required.

6. Click Save when finished.
