Medical History Recorded Class
Enter medical history items directly into a patient record, or download a printable .PDF version of the questionnaire for the patient to complete. Each time a patient's medical history is updated, an entry is created recording the date and staff member who updated the record.
Note: Beginning in April 2021, Medical History information is collected via custom Patient Questionnaires. The most recent Medical History templates have been automatically converted to the new process. (Medical histories collected using older versions of the template remain as read-only records in the patient's Medical History UI.) All existing questions and items have been created in the Question Library and assigned to the Medical History category. The Medical History template has been created in the Questionnaire Template Library, assigned to the Medical History category, and named Medical History SC.
To collect medical history data:
- From the Patient tab, select Medical History. The Medical History window is displayed.
- Under Actions, select Collect New Medical History. The Questionnaires window is displayed.
- Under Actions, select Collect Data. The Select Questionnaire Template window is displayed.
Note: The Collect Data function can also be accessed from the Patient Homepage, the Hard Tissue Chart (US)/Dental Chart (UK), and the Orthodontic Clinical Summary. - Select your Medical History template (either Medical History SC, or the preferred questionnaire template created/edited for this purpose).

You are prompted to choose how to proceed. - Do one of the following:
- Select Print Blank Questionnaire to save the form as a .PDF that you can send to the patient ahead of an appointment or print for the patient to complete in the waiting room.
- Select Collect Responses to complete the questionnaire on screen. - If you select Collect Responses, use the fields in the displayed questionnaire to collect the patient's medical history. You can also scroll down and:
- Add comments in the Notes field.
- Select Copy Previous if a previous version of this questionnaire exists.
- Select Yes to All or No to All, as appropriate. - Click Save Responses. The Medical History is saved in the patient's list of questionnaires.
Note: If the selected questionnaire was designated as a Clinical Event, you cannot edit it beyond the current day. To update a previous questionnaire, select Copy As New to create a new questionnaire that contains the previous answers.
Note: Printed forms completed manually can be scanned into the patient's record and saved using the Patient Files feature.
