If a patient has multiple Responsible Parties in the Patient Record, the patient statement is addressed to the Responsible Party who has the Guarantor indicator (green star) selected as shown below.

If the patient is a minor, the statement is addressed with Patient Name c/o RP-Guarantor, as shown below:
